SUPPLEMENTAL EMPLOYMENT FORM
Complete all information and sign in the appropriate areas.
FORM I-9 - EMPLOYMENT ELIGIBILITY VERIFICATION
Complete this form NO LATER than on the first day of employment. Follow the instructions to complete Section 1 (page 7). Within three days of your employment, you will be required to provide documents verifying your identity and U.S. employment eligibility, as shown in Section 2 and the attachment to the I-9 form. Remember to bring these documents with you to work.
BANK DEPOSIT AUTHORIZATION FORM (Electronic Direct Deposit)
Direct deposit of payroll checks is mandatory. When submitting the paper form, for account verification, a voided check or deposit ticket must be submitted as noted on the form.
DRUG POLICY STATEMENT (AGCENTER PS-18)
Read AgCenter PS-19 "Illegal Use of Drugs or Alcohol" and sign the certification statement. Submit the certification statement with your employment forms. Keep the PS-18 and one copy of the certification statement for your records.
CAR INSURANCE AGREEMENT - (For Cooperative Extension Academic Employees Only)
Sign and return form.
NAME TAG INFORMATION
Complete and sign if you will serve in a capacity to represent the AgCenter away from the office.
Ethics, Civil Rights, and Sexual Harassment Prevention Education is mandated for all Louisiana public employees.
NEW EMPLOYEE ORIENTATION
Please complete and return the second page of this form to HRM office as indicated. ALL NEW EMPLOYEES (over 120 days) MUST ATTEND.
NEW EMPLOYEE ORIENTATION CHECKLIST
This is required for all new employees. Please have a supervisor go over the items with you. If you are attending a benefits info session, HR can sign off on that portion. Once completed, please return to HRM.
